Terms and conditions for Platinum | Unlimited Membership
- All Class Plans
require a 3 month FIT4MOM Oak Park River Forest commitment. Any cancellation of a plan prior to
the 3 month commitment timeframe will result in a $50 Early Cancellation Fee.
To cancel your membership you must complete and submit our Membership
Cancellation/Hold Request Form at least 30 days prior to the 1st of the month
in which you wish to cancel your membership to avoid payment.
- There are no
refunds on your monthly membership once monthly payment has been processed. If
client bought at the Founder's Rate and chooses to cancel or modify the plan,
the client will lose the Founder's rate and will need to purchase at full price
if she decides to return or rebuy.
- Qualifying
memberships can be placed on hold for no less than 30 days. Monthly Membership
Plans will not be prorated for vacations/holidays. A maximum of one hold is allowed per calendar year (Jan-Dec). There is a $15/month hold fee for
non-medical hold requests. You must complete and submit our Membership
Cancellation/Hold Request Form 30 days prior the 1st of the month in which you
wish to hold your membership to avoid automatic monthly payment. There are no
refunds on your membership once the monthly payment has been processed.
- There is a
one-time $60 enrollment fee for all new memberships unless a promotion is being
offered. Members who transfer to us from another FIT4MOM franchise will not be
charged an enrollment fee.
- FIT4MOM OnDemand Monthly Subscription: With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly cancelled according to the Terms and Conditions, your OnDemand subscription will be automatically cancelled.
